Estrogen and Migraines: What Bloodwork Doesn't Show
If you've had bloodwork done and seen the number telling you your estrogen is off, I believe you. I'm not here to tell you your labs are lying. What I want to show you is what that number can and can't actually explain, because that distinction changes everything about what you do next.
What Estrogen Actually Does
Most of us were taught estrogen is just a reproductive hormone, and that's about where the education stopped. But estrogen has a real relationship with your nervous system. It affects serotonin, it affects your pain receptors, and it plays a role in keeping your nervous system calm and regulated. When estrogen drops, even briefly, your pain threshold drops right along with it. That's not in your head. It's a measurable shift in how your brain processes pain signals.
This is why migraines often cluster around specific times, not because estrogen is too high or too low in some fixed sense, but because it's the fluctuation, the drop itself, that your nervous system reacts to.
Perimenopause, Postmenopause, and What Changes
If you're perimenopausal, this same fluctuation is happening, just less predictably and over a longer stretch of years. That's part of why perimenopause can feel like your migraines have a mind of their own.
Postmenopause is a different chapter of this same story. Once your periods have stopped for good, estrogen settles into a lower, steadier state. For some women, migraines actually improve here, because the wild fluctuation that was driving pain has quieted into something more level, even though the level itself is lower than before.
I want to be really clear here. It is completely normal for estrogen to be different at 45 than it was at 25, and different again at 55. Your hormones aren't supposed to stay frozen at one setting your whole life. At sixteen, estrogen cycles in a fairly predictable rhythm. In your thirties, that rhythm is usually still steady. In perimenopause, it starts swinging more widely and less predictably. In postmenopause, it settles low and stays there. Every one of those stages is normal.
What isn't the problem is that estrogen changes as you age. What is the problem is the size of the swing and how fast it happens.
Your nervous system doesn't respond well to sudden change. It responds to rate of change. A slow, steady decline gives your brain time to adjust its baseline. A sharp drop, the kind that can happen within days during perimenopause, doesn't give your nervous system that adjustment window. Serotonin drops right along with it, and pain receptors become more reactive almost overnight.
The Piece Almost Nobody Talks About
Your liver has a job most of us never think about: clearing hormone waste out of your system. Once estrogen has done its job, it needs to be processed and cleared, mostly through your liver and gut working together. When that clearance process isn't working well, because of inflammation, constipation, poor digestion, or an overloaded liver, old estrogen doesn't leave the way it should. Your body can actually behave as if estrogen levels are higher or more erratic than they really are, simply because clearance is backed up.
This is why I gently push back on relying too heavily on a single blood test. Bloodwork has real value, I'm not telling you to stop testing. But estrogen fluctuates by the hour, your liver's clearance capacity isn't captured on that test at all, and a single draw on a single day is one frame out of a very long movie. When we lean too hard on that one frame, we can miss what's actually driving the pain, often the inducers underneath all of it: inflammation, sleep, blood sugar, and stress.
A Real Example
A woman in our community realized she'd gotten her period and been away for the weekend on a road trip, and didn't get a migraine. Either situation would typically leave her with a headache or worse, and this time, neither did. That's not luck. That's what it looks like when the ground underneath estrogen, the inflammation, the inducers, is actually more stable.
Estrogen isn't the villain here. It's doing what estrogen does, and it's supposed to change as you move through different stages of life. The real question is whether everything underneath it is stable enough to handle the normal ups and downs that come with being a woman in a body designed to fluctuate.
