Sex Hormones and Migraines: Why Advice Contradicts

If you've been told your migraines are hormonal, you've probably also been given a handful of different explanations for what that actually means. Your gynecologist wants you on HRT. Your neurologist isn't so sure. Your naturopath wants bloodwork done. And somewhere in the middle of all of it, you're left holding a lab result and still not knowing what to do with it.

Here's the first thing I want you to know. You are not confused because you don't understand your body. You are confused because you've been handed conflicting information from providers who each only see one piece of you, and you're smart enough to notice that none of it adds up.

A New Way to Picture Your Hormones

A while back, I recorded an episode on something I call the hormonal hierarchy. Since then, I've had hundreds of conversations with women in the Women's Migraine Freedom™ group and on assessment calls, and those conversations taught me there was a lot more to say. So I want to give you a clearer picture, one you can actually hold onto.

Picture a triangle. At the very tip, the small point at the top, sit your sex hormones: estrogen, progesterone, testosterone. That's what gets tested. That's what gets a prescription. That's what gets blamed. But a tip can't stand on its own. It needs a base holding it up. And that base is made up of oxytocin, cortisol, insulin, and your thyroid.

What Each Hormone in the Base Actually Does

Oxytocin is your connection hormone, released through touch and closeness, and it quietly supports insulin sensitivity and helps regulate sex hormones in the background.

Cortisol is your stress hormone. It's supposed to rise and fall throughout the day, but when it stays elevated for months or years, it directly suppresses progesterone production.

Insulin manages blood sugar, and because it's a major hormone, your body genuinely cannot balance estrogen, progesterone, or testosterone until insulin is working properly first.

Your thyroid runs your metabolism and touches nearly every organ you have, including your ovaries. When it's sluggish, your whole reproductive system slows down with it.

Why an Unstable Foundation Doesn't Always Look Dramatic

If the base is unstable, if cortisol is running high, if insulin is out of balance, if your thyroid is sluggish, the tip at the top is going to wobble no matter what you put on it or in it. You can add estrogen, add progesterone, try every cream and patch on the market, and it will keep wobbling because you're addressing the tip of a triangle with no foundation underneath it.

Sometimes this shows up quietly. You wake up and your joints ache a little more than usual. Your mood sits closer to the surface than normal. Your sleep has been lighter. There's a low hum of anxiety that wasn't there a few years ago. And then a migraine shows up, and it feels like it came out of nowhere, except it didn't. Your body had been telling you the base was shaky for weeks.

I want to be clear about something too. All of these hormones are supposed to shift over time. That's not a malfunction, that's a body that's alive and adapting. The problem was never that your hormones change. The problem is when the base gets so unstable it can't hold the tip steady anymore.

If you've tried the patch, the pill, or HRT and it hasn't held, I don't want you thinking something is wrong with you. I want you understanding that you've been working on the tip of the triangle this whole time, and nobody ever showed you the base.
